I authorize DC Communications to initiate recurring, automatic electronic fund transfers (“automatic transfer”) from the credit card account (“Account”) indicated above to pay my DC Communications monthly invoice. As I selected above, the amount of the automatic transfer from my credit card will equal the Total Amount Due on my DC Communications account on the date my payment is due.
I understand that DC Communications will process my request upon receipt of this form and send me written confirmation as to when the automatic transfers will begin. Until I receive written confirmation from DC Communications, I understand that I should continue making my payments by credit card or check. The automatic transfer will be deducted from my Account on the payment due date shown on my invoice. This authorization will remain in effect until I order my bank to stop payment or until DC Communications notifies me that this automatic transfer is no longer in effect. If I decide to stop payment, I agree to notify DC Communications when I do so. DC Communications may cancel this agreement if: (a) the financial institution where I have my Account fails for any reason to honor any DC Communications automatic transfer request; (b) my Account has insufficient funds to pay any DC Communications automatic transfer request; (c) any other reason deemed sufficient by DC Communications. If my financial institution refuses the automatic transfer for any reason, any credit for the payment that DC Communications makes to my DC Communications account will be reversed. If the automatic transfer is refused by my bank for any reason and consequently DC Communications does not receive my credit card payment on time, DC Communications may also charge my credit account a Late Payment Fee in the amount specified in DC Communications pricing list or local tariff (see here for pricing list or local tariffs), as amended to date. The terms and conditions of my DC Communications pricing list or local tariff will continue to apply, including any applicable fees and finance charges.
